I recently made the switch to EFA, and open-source Email Filter Appliance.

Took some time to set up and tweak to my liking, but I'm very happy with it now.
It basically does the same thing as CGPSA-SA + CGPAV-ClamAV, but on a separate (VM) box, and features a lot of bells and whistles (greylisting, DMARC, DKIM, AD support, quarantine retention,...)
